Fisher Plaza Forensic Report Update

Fisher Plaza has released the official forensic report from the Plaza East electrical failure event on July 2. The event resulted in extensive damage to two 5000 A bus ducts connecting the utility source equipment located in an adjacent electrical vault to the building power switchboards in the electrical room. The investigators concluded that:

The most likely cause of the incident is the failure of the insulation between phase C bus and ground in bus duct 1 as the result of progressive degradation of the insulation or the structural integrity of the assembly. Bus ducts consist of insulated bus bars that are sandwiched together to form a compact bus way system. In areas where two individual sections are joined together or where the bus ducts form a bend, a system of transition buses and through bolts are used to help secure the joint or the bend. These areas are susceptible to failure as they are under increased thermal and mechanical stresses. More often failures are found at such locations of the bus duct systems. It is our opinion that this was the most likely cause of failure.

Adhost continues to work closely with Fisher Plaza and its vendors on the long-term solution that will take this event into consideration and will allow Adhost to operate continuously through a similar loss. This solution includes the following:

  1. Diversifying the Adhost East critical equipment loads across the two switchgear rooms that service or will service Fisher Plaza East.
  2. Working with Fisher Plaza to ensure that catastrophic loss of one room will not inhibit our ability to operate.

We are on track to have the replacement for the damaged equipment in place by the end of October. We will update you if there are any changes to this schedule. We do not have a timeline yet for item #2 above.  Although significant progress has been made, Fisher and its vendors are still refining options and defining budgets for this item.
